PHILLY CEAL

THE PHILADELPHIA COMMUNITY ENGAGEMENT ALLIANCE

Philly CEAL is a city-wide collaborative effort aimed at improving the health and well-being of Philadelphia communities. Philly CEAL is part of the NIH CEAL research network, which is designed to identify promising engagement and outreach practices that communicate trustworthy, science-based information to communities experiencing health disparities.
 
We are delighted to announce that Philly CEAL will launch a new 4-year program, in partnership with the University of Pennsylvania’s School of Nursing, the City of Philadelphia’s Office of Community Empowerment and Opportunity (CEO), and our Community Advisory Coalition. 

What does Philly CEAL aim to do?

Aim 1
Implement and evaluate the real-world effectiveness of a group-delivered Community Health Worker (CHW) program to improve chronic disease self-management.
Aim 2

Promote collaborations between multiple sectors to promote new opportunities to improve prevention and care service delivery.

Aim 3

Address the social determinants of health that drive health disparities and inequities in chronic disease self-management.
